I could not find one either so I made one with the modifications I have done on my board.
The layout of the board is just to show where the components are.

May I know which tool did you use to make schematic and layout?

I've updated the drawing with the missing Vcc and attached pictures showing my modification.
The schematic is drawn using Eagle from CadSoft.
