I have the same question. Do we put a compiled .bin file on the SD card? Any name or a particular name the file should have? Root or some folder?

Anybody successfully booted ESP from an Sd card?

How about connecting the SD card, booting the bootloader and flashing an image, then inspect the SD card contents?

You can not boot from SD. Those ports (same as for the flash) are used to connect the ESP as SDIO device through the SDCARD bus, meaning that the computer/device should be able to control and use the ESP though that interface.
The specs and communication are yet to be revealed.
I used to have such wifi device giving me network access on my old Dell PDA

I don't think the SD-card is supposed to have a file system on it. Just blocks, start at 000000.
